The response: "1. Please provide questionnaires/interview schedules. 2. Obtain written consent. Please provide a detailed consent form. 3. Please specify a date for the destruction of the data; include this information in the consent form." Ahhhh. Here we go again! Just tried this with my own IRB. The response:
